Meeting Room Request Form

  10. You may bring refreshments (no alcoholic beverages). While we will do our best to have everything set up to your specifications, we cannot guarantee room setup, especially on weekends. Feel free to move furniture to suit your needs, but please leave the room in the condition you found it in.

DISCLAIMER The Library assumes no responsibility for any equipment, supplies or materials brought to the Library by any group or individual attending the meeting; nor does it assume any liability for groups or individuals attending a meeting in the Library. For, and in consideration of the use of the meeting room, any person or group using a meeting room shall agree to indemnify and hold harmless the Mt. Lebanon Public Library and its representatives from and against any and all claims, demands, judgments, settlements, damages, actions, causes of actions, injuries, administrative orders, consent agreements and orders, liabilities, penalties, costs and expenses of any kind whatsoever, which may arise or be asserted, directly or indirectly, with regard to the use of the room. The Library assumes no responsibility for damage to, or theft of, any item displayed or exhibited in the meeting room. All items are placed in the meeting room at the owner’s risk. If audio or visual equipment is used, the user shall set it up in advance of the meeting hour. The user shall have knowledge of how to operate the equipment. It is not the responsibility of the Library staff to operate the equipment. Any audio-visual presentations require proof that public showing is not in conflict with current copyright laws. If you are providing your own equipment, you are responsible for the set up, operation, and trouble shooting of your equipment. I understand that the meeting rooms may not be used for sales, solicitation or other commercial purposes. I realize that failure to give 25 hours notice of cancellation will result in a ban on room use for a minimum of six months. I agree to report any injury or accident occurring on the premises. (I agree to abide by these and all other terms and conditions as set forth in the Mt. Lebanon Public Library Meeting Room Policy #13, and hereby acknowledge receipt of a copy of said policy.)
